Registered participants in the Annual Clinical Care Options Symposium are eligible to take advantage of our specially negotiated discount room rate of $229 per night, plus tax. This special rate will be honored between June 8-11, 2012. Accommodations before June 8 and after June 11 are subject to hotel availability. This rate is based on double occupancy.
To take advantage of the negotiated discount rates, you must register online or complete the hotel information form and remit your full payment (checks should be made out to The Westin Diplomat) to the address on the bottom of the form. Reservations received after March 31, 2012, will be accepted on a space-available basis. For more information, please call the registration team at 703.674.3569.
To guarantee hotel accommodations, you must include payment for 2 room nights ($229 per night) plus 11% tax for a total of $508.38.
Please return this payment along with your registration form. If you are paying by credit card, your card will be charged by the hotel in full on or around March 31, 2012. All hotel reservations are nonrefundable after March 31, 2012.
All hotel reservations for the symposium must be made through the CCO Registration Office.
Fees, less a $100 processing charge, are refundable if the registration team receives your cancellation no later than March 31, 2012. All cancellations must be in writing.
Substitution of another qualified registrant may be accepted upon receipt of a newly completed registration form.
No refunds will be given after March 31, 2012. If you are paying by credit card, your card will be charged (by the hotel) in full on or around March 31, 2012.
